Please do not submit symptoms, diagnoses, medication details, photographs, insurance information, Social Security numbers, lab results, or other protected health information through general website forms. Medical information should be submitted only through a secure patient process, patient portal, telehealth platform, or other approved clinical workflow.
Telehealth
Telehealth terms
Telehealth visits are offered when clinically appropriate. Some concerns may require in-person evaluation, physical examination, laboratory testing, imaging, urgent care, emergency care, or specialist referral. Precision Primary does not guarantee that a telehealth visit will result in a prescription, referral, medical note, medical documentation, or a specific treatment.
Patients are responsible for using telehealth services from a private location with a reliable internet or phone connection. If the provider determines telehealth is not appropriate, an in-person visit or higher level of care may be recommended.
SMS and email
Messaging consent

Precision Primary may establish cancellation, late cancellation, or no-show policies. Any applicable fee should be communicated during scheduling or before service is provided.
Prescriptions
Medication and prescription policy
Prescription decisions are made by the licensed provider based on medical history, clinical evaluation, applicable laws, safety considerations, and professional judgment. A requested medication, refill, dose change, GLP-1 medication, hormone therapy, antibiotic, or other treatment is not guaranteed.
Some medications may require records, labs, ongoing monitoring, in-person evaluation, specialist involvement, or follow-up visits. Compounded medications are not FDA-approved and are not reviewed by the FDA for safety, effectiveness, or quality before marketing.
Memberships
Membership terms
Memberships are not health insurance and do not replace health insurance. Membership fees cover only the services specifically listed in the selected plan. Labs, imaging, medications, supplements, vaccines, specialist care, emergency care, hospital care, and other third-party services are not included unless expressly stated.
Membership benefits, visit limits, billing terms, cancellation terms, and included services should be reviewed before enrollment. Precision Primary may update membership offerings, pricing, and included services as the practice develops.
Mobile primary care
Mobile care terms
Mobile visits are available within the designated service area and are subject to scheduling availability, travel fees, safety considerations, and clinical appropriateness. Precision Primary may recommend telehealth, in-office care, urgent care, emergency care, or specialist referral when medically appropriate.
Mobile visits may not be appropriate for emergencies, advanced procedures, severe symptoms, unstable medical conditions, or situations requiring immediate testing, imaging, hospital-level care, or emergency services.
